All families must update their child’s Health Tracker status the day of each event, preferably 1-2 hours before that event.
The survey MUST be completed the same day of that practice or game; failure to follow this rule may result in your child’s suspension from participation.
The survey consists of six questions. Due to the nationwide use of this app and the varying requirements of states and organizations within each state, these questions are hard-coded and may not be fully applicable to the club's protocols, which were established in conjunction with the county's health department.
In the event your child’s survey fails due to user error (fat fingers, too quick to submit, etc.): notify your team’s coach, so they may override the result and permit your child to participate.

* If the child is showing symptoms of or has tested positive for COVID-19
* If a household member is showing symptoms or has tested positive for COVID-19.
* If the child was in close contact of someone showing symptoms of or diagnosed with COVID-19
* Relevant dates and details related to these scenarios.
The club’s Safe Return to Play Committee will review these details and may request additional information to determine if the child must pause their participation.
