You must complete both of the forms below and read the referee assignment information before working for Loudoun Soccer. Once complete, the forms should be mailed to Deborah Baumgardner at the address on the referee data form.

Referee Data Form (Word)
This information is needed to get you set-up in out referee database.

W-9 Form (PDF)
Print and complete page 1 only. Pages 2-4 are not necessary.  Make sure you sign and date this form. Incomplete forms will be returned and delay the process of getting you set-up.  This is required by the IRS. If you earn more than $599 per year, you will receive a 1099 form for your taxes.
